Eldercare Referrals
The EAP eldercare specialists gather information about your eldercare situation in order to determine which services are needed. An updated database of eldercare resources is maintained for use in arranging referrals.
The EAP care specialist will save you time and give you peace of mind by assessing the appropriateness of facilities and ensuring that there are available openings before providing you with any referrals.
Referrals are available for the following services:
- Care Options and Living Arrangements
- Caregiver Issues and Concerns
- Professional Geriatric Care Management
- Meal Delivery and In-home Services
- Transportation Services
- Hospice Services
- Medicare, Medicaid and Social Security
- Respite Care
- Funeral Planning
In-Person Eldercare Services
EAP's in-person eldercare services are provided by Geriatric Care Managers (GCM). GCMs are professional social workers or nurses who are trained in assessment, care planning and care management of adults with special needs, ill spouse/partner or older adults. This program provides free access nationwide to GCMs for up to three hours of geriatric care management services per employee per year.
The following services are provided:
- In Home Care Assessments - A GCM provides an assessment of the care recipient's home and activities of daily living and delivers a detailed care plan.
- Facility Reviews- A GCM will tour selected care facilities (assisted living and nursing home facilities) with client to evaluate and review with you the environment, care, staffing and overall level of quality.
- Hospital and Post-Hospital Assessments- A GCM will meet clients at the hospital prior to the care recipient's discharge to evaluate his/her condition and needs. In many cases, the GCM can evaluate the home or facility where the care recipient will be going. After the discharge from the hospital or rehab facility, the GCM can visit the home and monitor the care.
- Ongoing Case Management and Care Coordination- A GCM provides a variety of services including care coordination of medical services, bill paying, appointment coordination, pharmacy pick-ups, and arranging other needed community-based services.
- Detailed Care Plan- The GCM provide clients with a detailed care plan. In this care plan, the loved one's current situation, past history and future needs are documented and detailed. The GCM identifies areas of concern that need to be addressed and offers a comprehensive written plan of resources to consider and next steps to take. Depending on the needs, the GCM will provide the client with updated local resources and will facilitate referrals and placements if needed.

CBP Backup Care
The CBP Backup Care Program offers you and your family more family care options. This program will help employees find and pay for temporary short-term center or in-home care so you can be at work when your regular child/adult/elder/pet care is unavailable.
Access to high-quality backup care helps you get to work with the confidence of having a reliable, emergency backup provider at a cost you can afford.
Free Adult Caregiver's Kit
If you are caring for your aging parent(s) or another adult with care needs, the free Adult Caregiver's Kit provides information and products to support your caregiving and give you peace of mind. The kit includes several informational guides and a book to help you manage your eldercare responsibilities. Call the EAP to request your free kit today.
Taking Care of Yourself
Caring for an elderly relative can be very stressful. Your Employee Assistance Program provides employees and their eligible family members with up to 12 free face-to-face counseling sessions per year. Counselors can help employees cope with the challenges of providing care for an elderly relative and can help you find support groups to ensure you are linked with members in the community who are facing similar life challenges.
